class="wp-block-paragraph">A moment earlier, she had threatened lawsuits and demanded antibiotics so she could take her son home.

Now two security guards were holding her against the wall, and she was staring at the cast saw as though it were about to expose something she had spent weeks hiding.

I looked at Clara.

“Keep his arm still.”

Marcus moved to the head of pulse.

“His name is Ethan,” Martha said suddenly. “You keep calling him ‘the boy.’ His name is Ethan.”

I placed the saw against the filthy fiberglass.

The blade vibrated instead of spinning, designed to cut hard casting material without slicing skin, but Ethan still flinched when it touched his arm.

His eyelids fluttered.

“Easy,” I said. “You’re safe right now.”

His lips moved beneath the oxygen mask, but no sound came out.

The first cut released a pocket of air so foul that Marcus turned his head and gagged into his shoulder.

Clara’s eyes watered above her mask.

I kept moving.

The fiberglass had been repaired in several places with strips of gray duct tape, and beneath one layer I found a grocery bag twisted around the padding as a crude waterproof liner.

It had trapped heat and moisture against the child’s skin.

When I opened the second side of the cast, the shell separated with a wet tearing sound.

Something pale dropped onto the sterile blue drape.

Then another.

Then dozens.

Small larvae spilled from the rotting padding and scattered across the floor beneath the trauma bed.

Clara screamed and stumbled backward into the instrument cart.

Marcus froze with an IV flush in his hand.

One of the security guards swore under his breath.

Martha closed her eyes.

I did not have time to react.

“Specimen container,” I said. “Now.”

<div class="google-auto-placed called for the surgical team and repeated Ethan’s vital signs over the phone.

The skin beneath the padding was swollen and discolored, with areas where infection had spread around his wrist and forearm.

His fingers remained cold and blue.

There was almost no detectable pulse at the wrist.

The smell grew stronger once the cast was completely open, but the most frightening thing in the room was not the odor or what had fallen out.

It was the evidence that Ethan had been living with this while someone kept covering it back up.

I found folded dryer sheets stuffed between layers of damp padding.

More had been pushed near the opening above his elbow.

Someone had tried to perfume the cast instead of treating what was happening underneath it.

“Who put these in here?” I asked.

Martha shook her head quickly.

<p
